Requests must be made with this specially produced offer certificate may not be mechanically or otherwise reproduced. Once your name and address has been printed on the offer certificate you cannot change your name or address. Requests from and shipments to PO Boxes will not be honor.

Any alterations to the offer certificate will make this offer void.

Requests must come from individual consumer. Please allow eight weeks to receive your rebate check. Limit one request per household or address. Trading of proo-of-purchase is strictly prohibited. This offer is limited to the United States only. Requests from P.O. Boxes will not be honored, except in Alaska. Use of fictitious names, addresses, or organizations to obtain additional rebates is fraud and is in violation of Federal Law, and may result in prosecution and a term of imprisonment.
